Be sure to include the following information when you contact us:
- Patient Name
- Date of Birth
- Patient Telephone Number
- Physician’s Name Ordering Medication
- Name and Dosage of the Medication
- Any Drug Allergies
- Name and Phone Number of the Pharmacy
Medications will not be refilled from 4:00pm to 8:00am Monday through Friday.
*Medications called in after 3:30pm on Friday will not be filled until the following Monday.
Once your medication request is processed, please contact your pharmacy for pick-up instructions.
